Collom & Carney Clinic Association and Humana Establish Accountable Care Agreement to Improve Seniors’ Health and Wellness

Achieving the Triple Aim – improved medical outcomes, a better patient experience and lower costs – is the primary objective behind a new three-year Accountable Care agreement between Collom & Carney Clinic Association and Humana Inc. (NYSE: HUM).

The agreement, effective December 1, 2014, will restructure health care delivery for Humana Medicare Advantage members in Texas and Arkansas to improve access to Collom & Carney Clinic’s large network of physicians, improve standards of care, enhance health care coordination and improve wellness opportunities.

The accountable care partnership offers Humana’s Medicare Advantage Health Maintenance Organization (HMO) and Preferred Provider Organization (PPO) membership in-network benefit access to primary and specialty care through Collom & Carney’s 90-plus physicians practicing at 11 clinics in Texarkana, New Boston, Wake Village and Prescott, AR.

Accountable care is a fast-growing model for health care service delivery and health care payment. The pay-for-value system is focused on evidence-based, highly-integrated care, which can lead to the Triple Aim.

Humana members will have access to an integrated-care team approach that offers better care coordination with a goal of improving quality outcomes and quality of life. The physician-led teams, depending on the practice, include an array of providers and services, for example, nurses, health coaches, medication management, behavioral health and social services resources.

The Humana and Collom & Carney Clinic partnership emphasizes quality outcomes with a focus on care coordination and leverages measures as defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) like Comprehensive Diabetes Care, breast cancer screenings, colorectal cancer screenings and high-risk medication. The partnership offers a broad range of population management tools like predictive analytics, chronic disease management and wellness programs.

Humana has a 27-year accountable care relationship history with more than 1.3 million individual Medicare Advantage members today that are cared for by 42,300 primary care physicians, in more than 900 accountable care relationships across 43 states and Puerto Rico.
