During the vasectomy reversal surgery, after local anesthesia has been provided, a small incision is made on both sides of the scrotum. This will allow Dr. Hickman to find the two ends of the vas deferens. He will then free those ends from the surrounding scar tissue. Next, he will perform a microsurgical repair of the vas deferens using a 10-0 nylon suture and a Zeiss OPMI-MDM operating microscope.

To optimize the chances of success of the procedure, Dr. Hickman will get a sample of the fluid from the testicular end of the vas deferens, put it on a glass slide, and examine it under a laboratory microscope. This fluid should contain some sperm since the testicle continues to release sperm even after vasectomy. There are three possibilities with regards to what will be found in the sample fluid. The first possibility, which would be the best one, is that the fluid contains whole sperm. The second possibility is that the fluid is thin and doesn’t contain a whole sperm but only sperm parts. The third possibility is that the fluid is thick and pasty and doesn’t contain any sperm.

The third scenario indicates the presence of an obstruction, which is known as an epididymal blowout. The obstruction is typically found at two levels: at the location of the vasectomy and the rupture of a tube in the epididymis. This means that Dr Hickman will not only need to perform a vasovasostomy, but also a vasoepididymostomy. The vasoepididymostomy procedure will require a microscopic invagination of the epididymal tube into the lumen of the vas deferens and this will also need 10-0 nylon sutures. But patients need not worry because there will be no additional cost for them if both repairs need be done for the vasectomy reversal.

Dr. Hickman obtained his medical degree from the University of Texas Medical Branch in Galveston, Texas. He then trained in General Surgery Residency for five years at the University of Texas Health Science Center at San Antonio and it was during this period of time that he came to know about the microsurgery techniques being applied by plastic and reconstructive surgeons.
