Diagnostic imaging procedures involving contrast media - iodine- or gadolinium-based solutions that are injected intravenously to help radiologists see organs more clearly - require supervision by qualified medical personnel, as the substances carry the risk of severe allergic reactions.
In the pre-pandemic era, CMS and ACR guidelines stated that a supervising physician must be “immediately available” to provide guidance in the event of adverse reactions; this definition meant that the physician had to be physically on-site during the time of the procedure.
However, the CMS has now amended its definition of “direct supervision” to allow for remote supervision via two-way, real-time audio-video technology, permanently authorizing the virtual supervision model for level 2 diagnostic tests, effective January 1, 2026. The amended rule fundamentally transforms how radiology departments can carry out contrast-enhanced imaging procedures: qualified practitioners may now meet the “immediately available” requirement remotely, eliminating the need for on-site presence.
Experts like ContrastConnect say that this rule change stands to benefit medical facilities, especially those located in underserved communities. Imaging centers throughout the nation have struggled to meet growing patient demand amid the radiologist shortage, a crisis that has impacted rural areas disproportionately.
With the virtual supervision model, a single radiologist can oversee diagnostic imaging procedures in multiple sites without needing to commute, vastly improving staffing availability and potentially allowing facilities to extend operating hours and address appointment backlogs.
ContrastConnect’s guide notes that adherence to regulatory guidelines for the virtual supervision model includes remaining up-to-date on federal, state, and local healthcare laws and maintaining thorough documentation, clear emergency protocols, ongoing staff training, and transparent communication with related parties. Patient safety is also paramount, and independent outpatient imaging facilities that adopt the model must install the appropriate infrastructure, which includes HIPAA- and HITECH-compliant technology, and ensure that redundancies are in place in the event of outages.
